
Anxiety disorders encompass a set of related conditions characterized by excessive fear, worry, and behavioral or somatic manifestations that impair functioning. While anxiety can be adaptive, pathological anxiety involves intensity, duration, and frequency disproportionate to actual threat, often persisting despite reassurance. Common anxiety disorders include generalized anxiety disorder (GAD), panic disorder, social anxiety disorder, specific phobias, and agoraphobia, each with distinct patterns of triggers and symptom clusters.
Neurobiologically, anxiety reflects dysregulation within fear circuitry and stress-response systems. Key structures include the amygdala (salience and threat detection), hippocampus (contextual learning), and prefrontal cortex (top-down regulation and cognitive control). In many patients, heightened threat sensitivity and impaired extinction learning contribute to persistent anxious states. Dysregulation in the hypothalamic-pituitary-adrenal (HPA) axis is also implicated, with abnormal cortisol dynamics and altered stress reactivity affecting arousal, sleep, and attention. Neurotransmitter systems—especially gamma-aminobutyric acid (GABA) and serotonin—modulate inhibitory tone and threat appraisal, while noradrenergic pathways contribute to physiological hyperarousal (e.g., palpitations, sweating, tremor). Genetic vulnerability and early-life stress can shape these systems, increasing baseline reactivity.
Clinically, anxiety disorders present with overlapping but distinguishable features. GAD is marked by persistent, difficult-to-control worry about multiple domains (health, work, finances, relationships) accompanied by symptoms such as restlessness, fatigue, impaired concentration, irritability, muscle tension, and sleep disturbance. Panic disorder involves recurrent, unexpected panic attacks—abrupt surges of intense fear peaking within minutes—along with concerns about future attacks and maladaptive avoidance. Social anxiety disorder features fear of scrutiny or negative evaluation in social or performance situations, often leading to avoidance and safety behaviors. Specific phobias are characterized by marked fear of a circumscribed object or situation, provoking immediate anxiety and avoidance. Agoraphobia involves fear of situations where escape might be difficult or help unavailable, leading to avoidance patterns.
Diagnosis relies on careful assessment of symptom timing, severity, functional impact, and rule-out considerations. Clinicians evaluate whether symptoms are better explained by substance/medication effects, medical conditions (e.g., hyperthyroidism, arrhythmias), or other psychiatric disorders such as depressive disorders or obsessive-compulsive disorder. Standardized instruments (e.g., GAD-7, PHQ-9, panic and agoraphobia scales) can support screening, but diagnosis is fundamentally clinical. The DSM-5-TR criteria emphasize excessive worry or fear, associated symptoms, duration (often at least 6 months for GAD), and clinically significant distress or impairment.
Cognitive-behavioral models explain how anxiety is maintained by maladaptive thought patterns and behavioral learning. Catastrophic misinterpretation of bodily sensations (“heart racing means something is dangerously wrong”) can amplify fear and trigger avoidance. Anxiety leads to short-term relief through avoidance or reassurance-seeking, reinforcing the anxiety loop. Safety behaviors (e.g., carrying a “rescue” object, refusing to speak up) can prevent disconfirming evidence, sustaining false threat beliefs. Interoceptive conditioning and attentional bias further heighten physiological vigilance.
Evidence-based treatment is typically multimodal. First-line psychotherapy includes cognitive-behavioral therapy (CBT), which targets maladaptive interpretations, avoidance, and emotion regulation. For panic disorder, CBT may include interoceptive exposure to bodily sensations and cognitive restructuring. For GAD, CBT focuses on worry scheduling, cognitive restructuring, problem-solving, and behavioral experiments. Exposure therapy is central to phobias and social anxiety, using graded, therapist-guided confrontation with feared stimuli while preventing safety behaviors.
Pharmacotherapy is effective for many patients, particularly when symptoms are severe, persistent, or when psychotherapy access is limited. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly recommended due to favorable efficacy and tolerability profiles. Treatment response may require several weeks due to neuroadaptive changes. Benzodiazepines can reduce acute anxiety symptoms through GABA-A receptor modulation, but they are generally used short-term or selectively because of tolerance, dependence risk, sedation, and withdrawal phenomena.
Adjunctive strategies can enhance outcomes: sleep regularization, exercise, stress management, and reducing substances that can worsen anxiety (e.g., excessive caffeine, stimulants). In refractory cases, specialty approaches such as intensive CBT, augmentation strategies under psychiatric supervision, or neuromodulation may be considered. Because anxiety disorders frequently co-occur with depression, clinicians assess comorbidities to optimize care.
Early identification and sustained treatment are associated with better prognosis. Patients benefit from psychoeducation about threat-processing mechanisms, consistent therapy engagement, and relapse-prevention planning. If symptoms include severe functional impairment, suicidal ideation, or medical red flags (e.g., chest pain with exertion), urgent medical evaluation is warranted to exclude non-psychiatric causes.
