While it is commonly taught as a stress-reduction technique, its effects map onto well-established mechanisms relevant to anxiety, insomnia, and autonomic dysfunction.

From a mechanistic standpoint, slow breathing modulates heart rate variability (HRV). HRV reflects the balance between sympathetic and parasympathetic (vagal) influences on the sinoatrial node. For insomnia, slow-paced breathing may facilitate sleep initiation by lowering arousal and improving parasympathetic dominance during pre-sleep periods.

Practically, effective diaphragmatic breathing typically involves inhaling through the nose, allowing the abdomen to expand, and exhaling slowly—often with a longer exhalation than inhalation. Clinically, longer exhalations may enhance vagal activity and reduce the urge to over-breathe. Sessions commonly last 5–10 minutes, performed once or twice daily initially, then adapted to tolerance and goals. The “mindful” element matters: attention is directed to sensation in the lungs and diaphragm, and distractions are acknowledged without escalation. This resembles interoceptive exposure principles used to desensitize feared bodily sensations when anxiety disorders involve somatic misinterpretation.

However, breathing techniques are not universally benign. In some individuals, especially those with panic disorder or high sensitivity to bodily sensations, forced slow breathing or breath retention can provoke discomfort or dizziness. Practitioners should avoid strategies that intentionally increase CO2 to extreme levels. Contraindications or precautions include severe chronic obstructive pulmonary disease with poor ventilatory reserve, significant cardiopulmonary instability, and situations requiring specialized respiratory care. A gradual approach is recommended: begin with comfortable, natural tidal breathing, ensure hydration and safety, and stop if symptoms worsen (e.g., chest pain, severe shortness of breath, syncope).

In summary, inhalation-and-exhalation breathing—particularly mindful diaphragmatic breathing with paced, gradual exhalation—can reduce stress and anxiety by modulating autonomic physiology (HRV/vagal tone), refining interoceptive attention, and altering threat-salience processing. It is best considered an evidence-informed behavioral skill that can support, rather than replace, comprehensive treatment when anxiety is persistent, impairing, or accompanied by panic, trauma symptoms, or major sleep disruption.
