Heat or cold therapy may reduce pain; heat can enhance muscle relaxation and blood flow, while cold may help with acute inflammation. For medication, nonsteroidal anti-inflammatory drugs (NSAIDs) can be effective for pain and inflammation when appropriate for the individual’s cardiovascular, renal, gastrointestinal, and bleeding risk profile. Acetaminophen may be used for analgesia, though anti-inflammatory effect is limited. Topical NSAIDs can be considered for localized musculoskeletal pain with potentially fewer systemic adverse effects.

Rehabilitation is central to recovery. Gentle, progressive mobilization—such as walking, trunk mobility exercises, and hip hinge practice—supports normalization of movement patterns and reduces fear-avoidance. Fear-avoidance beliefs and catastrophizing can amplify pain perception through heightened threat signaling and increased attention to bodily sensations, leading to reduced activity and delayed recovery. Cognitive-behavioral strategies and graded exposure to movement can mitigate this psychosocial component. Core stabilization exercises (e.g., controlled bracing, bridging) and stretching of tight muscle groups may improve function, although exercise plans should be tailored to symptom irritability.

Most cases of acute mechanical LBP improve substantially within days to a few weeks. However, if pain persists beyond the expected recovery window, recurrent episodes occur frequently, or function declines, reassessment is warranted. Persistent symptoms may require evaluation for disc pathology, spinal stenosis, or less common causes. Imaging such as MRI is usually reserved for those with red flags or persistent, function-limiting pain with suspected structural pathology.
