Insomnia and Sleep Deprivation: Neurobiological Mechanisms, Health Risks, and Evidence-Based Treatment Strategies

CBT-I typically includes stimulus control (reassociating bed with sleep by using the bed only for sleep and sex, and leaving the bed if unable to sleep), sleep restriction therapy (limiting time in bed to consolidate sleep, then gradually expanding it based on response), cognitive restructuring (addressing worry and unhelpful beliefs), and sleep hygiene education (regular schedules, morning light exposure, limiting alcohol close to bedtime, and reducing caffeine late in the day). Relaxation training and mindfulness-based strategies can be adjunctive when arousal remains high.

Pharmacotherapy may be considered when symptoms are severe or when CBT-I alone is insufficient. Options include short-term use of non-benzodiazepine hypnotics (e.g., “Z-drugs”), melatonin receptor agonists in select contexts, and sometimes low-dose sedating antidepressants for comorbid depression or chronic insomnia subtypes. Medication selection must account for age, comorbidities, fall risk, substance use history, and potential interactions. Clinicians generally aim to use the lowest effective dose for the shortest duration and to transition toward behavioral therapies when possible, given concerns about tolerance, dependence, and residual next-day impairment.

Emerging approaches include digital CBT-I programs, wearable-based monitoring, and personalized chronotherapy for circadian contributors. While these are promising, they still rely on core principles: reducing conditioned arousal, stabilizing sleep timing, and aligning circadian cues. Patients should be encouraged to seek evaluation for reversible contributors such as depression, anxiety, thyroid disease, medication side effects (e.g., stimulants, corticosteroids), excessive caffeine, alcohol-related sleep fragmentation, and restless legs triggers.

A practical clinical warning is that insomnia is both a symptom and a risk factor: it can emerge from stress or illness, but persistent insomnia can further amplify vulnerability to cardiometabolic and mental health conditions.
