Unlike transient, situational stress (e.g., excitement before an event), pathological anxiety involves maladaptive threat appraisal and sustained activation of the brain’s defensive systems. Clinically, this manifests as cognitive symptoms (rumination, difficulty concentrating, catastrophic interpretation), emotional symptoms (apprehension, irritability), and somatic symptoms (restlessness, muscle tension, insomnia, gastrointestinal discomfort, palpitations).

The neurobiology of anxiety centers on dysregulated circuitry involving the amygdala, prefrontal cortex, and hippocampus, with downstream effects on autonomic and endocrine systems. In anxiety disorders, this top-down regulation is impaired, increasing the likelihood that benign cues are interpreted as dangerous. The resulting sympathetic nervous system activation produces symptoms such as tachycardia, sweating, and tremulousness. Chronic stress exposure can also alter hypothalamic–pituitary–adrenal (HPA) axis function, affecting cortisol rhythms and contributing to persistence of symptoms.

Diagnostic categories include generalized anxiety disorder (GAD), panic disorder, social anxiety disorder, specific phobias, and anxiety related to trauma and other conditions. GAD is marked by persistent excessive worry occurring more days than not for at least several months, accompanied by symptoms such as restlessness, fatigue, difficulty concentrating, irritability, muscle tension, and sleep disturbance. Panic disorder involves recurrent unexpected panic attacks—abrupt surges of intense fear or discomfort peaking within minutes—often accompanied by chest tightness, dyspnea, dizziness, fear of losing control, or fear of dying. Social anxiety disorder features persistent fear of negative evaluation in social or performance situations, leading to avoidance or intense distress. Trauma- and stressor-related anxiety may include intrusive memories, hypervigilance, and avoidance patterns.

A key distinction is differentiating normal stress arousal from an anxiety disorder. Stress-related excitement typically is time-limited, tied to a clear trigger, and improves when the situation changes. Anxiety disorder symptoms are more pervasive, often occurring without a direct proportional trigger, and interfere with functioning at work, school, relationships, or health behaviors. Functional impairment is a major clinical criterion: individuals may avoid activities, rely on reassurance, experience reduced productivity, or develop secondary problems such as insomnia and substance misuse.

Cognitive factors play a major role. Many patients show attentional bias toward threat cues and cognitive distortions such as probability overestimation (‘‘this will go badly’’), severity overestimation (‘‘it will be catastrophic’’), and intolerance of uncertainty. Rumination reinforces negative meaning-making and maintains autonomic arousal. Interoceptive fear—fear of bodily sensations such as palpitations—can be central in panic disorder, creating a vicious cycle where anxiety symptoms trigger additional fear, leading to further physiologic escalation.

Treatment is evidence-based and typically multimodal. First-line psychotherapy includes cognitive behavioral therapy (CBT), which targets maladaptive thoughts, avoidance behaviors, and safety strategies. CBT for GAD often incorporates worry management, cognitive restructuring, and applied relaxation. For panic disorder, CBT includes interoceptive exposure to reduce fear of sensations and cognitive strategies to correct catastrophic interpretations. For social anxiety, CBT commonly uses cognitive restructuring plus exposure to feared social situations, often with performance-based components. Pharmacotherapy may include selective serotonin reuptake inhibitors (SSRIs) and serotonin–norepinephrine reuptake inhibitors (SNRIs), which modulate serotonergic and noradrenergic signaling implicated in threat processing and arousal regulation. Benzodiazepines can reduce acute symptoms but carry risks of sedation, dependence, and impaired cognition; they are generally used cautiously and for limited durations.

Lifestyle interventions can support recovery but are adjunctive. Regular sleep, aerobic exercise, caffeine and alcohol moderation, and mindfulness-based techniques may reduce baseline arousal and improve emotion regulation. Because anxiety can be exacerbated by medical conditions (e.g., hyperthyroidism, arrhythmias) or substances (e.g., stimulants), clinicians should consider a medical evaluation when symptoms are new, severe, or accompanied by red flags such as syncope, unexplained weight loss, or persistent chest pain.

When to seek help: if anxiety is persistent, escalating, or causing avoidance, impairment, or insomnia; if panic attacks occur repeatedly; or if symptoms include suicidal ideation. Early intervention improves prognosis. With appropriate therapy and medication when indicated, many individuals achieve substantial symptom reduction and improved quality of life.
