Depression in Adults: Clinical Features, Neurobiology, Risk Factors, and Evidence-Based Treatment Approaches

Depression in adults is a common, clinically significant mood disorder characterized by persistent low mood and/or loss of interest or pleasure, accompanied by cognitive, behavioral, and somatic symptoms that impair functioning. While transient sadness is normal in response to stressors, major depressive disorder (MDD) requires a symptom pattern lasting at least two weeks with sufficient severity to affect work, relationships, or daily activities. Clinically, depression is not merely an emotional state; it reflects altered brain network activity, neuroendocrine dysregulation, and changes in reward processing, which together produce a durable reduction in motivation, energy, and emotional responsiveness.

Core symptoms include depressed mood and anhedonia, but MDD also frequently involves sleep disturbance (insomnia, hypersomnia), appetite or weight changes, psychomotor agitation or retardation, fatigue, impaired concentration, excessive or inappropriate guilt, and recurrent thoughts of death or suicidal ideation. Diagnostic evaluation requires ruling out differential causes such as bipolar disorder, substance/medication-induced mood disorders, thyroid disease, anemia, and neurologic conditions. Screening tools—such as the Patient Health Questionnaire (PHQ-9)—aid detection but do not replace comprehensive clinical assessment.

Neurobiologically, depression is associated with dysregulation across cortico-limbic circuits, including the prefrontal cortex, amygdala, and hippocampal regions. Functional imaging studies commonly show altered connectivity in networks governing emotion regulation and cognitive control. Monoaminergic hypotheses have historically emphasized serotonin, norepinephrine, and dopamine; contemporary models integrate these pathways with neuroplasticity mechanisms involving brain-derived neurotrophic factor (BDNF) signaling, inflammation-related pathways, and glutamatergic dysfunction. Stress systems are also implicated: the hypothalamic–pituitary–adrenal (HPA) axis may exhibit hyperactivity or maladaptive feedback, contributing to sleep disruption, cognitive symptoms, and heightened negative affect.

Risk factors span biological, psychological, and social domains. A personal or family history of depression increases vulnerability, as does female sex, chronic stress, trauma exposure, and certain medical illnesses (e.g., chronic pain, cardiovascular disease). Older adults may experience depression in association with grief, social isolation, bereavement, or comorbidities; however, depression should never be dismissed as a normal part of aging. Cognitive vulnerability models explain how negative schemas, rumination, and cognitive distortions (e.g., catastrophizing, overgeneralization) maintain depressive symptoms by biasing attention toward adverse information and reinforcing hopeless interpretations.

Subtypes and related conditions include persistent depressive disorder (dysthymia), characterized by a longer, milder course; seasonal pattern with recurrent episodes during specific seasons; and postpartum depression, which occurs after childbirth. Bipolar depression must be differentiated because treatment strategies differ; antidepressant monotherapy can precipitate mania in individuals with bipolar disorder. Anxiety disorders, substance use disorders, and personality traits may co-occur, complicating diagnosis and treatment planning.

Evidence-based treatment is multimodal. Psychotherapies with strong support include cognitive behavioral therapy (CBT), which targets maladaptive thoughts and behaviors; interpersonal therapy (IPT), which focuses on role transitions and interpersonal disputes; and behavioral activation, which helps restore engagement and reward sensitivity. Pharmacotherapy may include selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), atypical antidepressants, or other agents depending on symptom profile and comorbidities. For severe, treatment-resistant depression, options include augmentation strategies (e.g., atypical antipsychotics in select cases), electroconvulsive therapy (ECT) for rapid symptom relief, and newer interventions such as ketamine or esketamine in appropriate clinical settings.

In addition to symptom reduction, effective management includes suicide risk assessment and safety planning. Clinicians evaluate access to means, intent, protective factors, and past attempts. Lifestyle interventions—regular physical activity, sleep regularity, and structured routines—can improve outcomes, particularly when integrated with psychotherapy and/or medication. Monitoring is essential: clinicians track symptom trajectories, adherence, side effects, and functional recovery, using standardized measures over time.

Ultimately, depression is a treatable disorder with well-established mechanisms and interventions. Recognizing symptom clusters early, addressing comorbid medical issues, and delivering evidence-based psychotherapy and/or pharmacotherapy substantially improve prognosis. Public discourse that targets individuals with harsh language may reflect interpersonal conflict, but clinical depression requires careful assessment and respectful, evidence-guided care rather than stigma.
