Limited access to mental health services and preventative care can prolong untreated anxiety and depression, while unmanaged stress promotes disease progression.
Clinical implications are substantial. Clinicians should consider economic stressors as part of a patient’s biopsychosocial assessment. Evidence-based interventions include trauma-informed and culturally responsive counseling, cognitive behavioral therapy (CBT) for anxiety and depression, and therapies targeting maladaptive stress appraisals. Pharmacotherapy may be indicated for moderate to severe anxiety or depression, but symptom control should be paired with addressing barriers to care. Sleep-focused interventions (behavioral sleep medicine, CBT-I), stress-management skills (mindfulness-based approaches), and supportive therapy can reduce arousal and improve coping.
At a population health level, reducing poverty-related stress requires upstream actions: stabilizing income, expanding affordable housing, improving access to healthcare and mental health services, and strengthening social safety nets. These measures can reduce HPA-axis overactivation, improve adherence to care, and buffer adverse cognitive and emotional trajectories.
In research terms, the stress-biodynamics model emphasizes mediation pathways—such as inflammation, HPA-axis dysregulation, and sleep impairment—linking chronic socioeconomic adversity to disease. The allostatic load framework describes cumulative physiological wear and tear from repeated adaptation to stressors. Higher allostatic load correlates with worse mental health outcomes and increased cardiometabolic morbidity.
In summary, poverty and economic insecurity can function as chronic stressors that alter neuroendocrine regulation, immune signaling, cognition, sleep, and health behaviors. This constellation increases risk for anxiety, depression, and cardiometabolic disorders through interacting biological and psychosocial mechanisms. Addressing both individual treatment needs and structural determinants is essential for effective prevention and care.
