Generalized Anxiety Disorder: Diagnostic Framework, Neurobiology, and Evidence-Based Treatment Approaches

Dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis may contribute to sustained hyperarousal and impaired stress recovery. Psychologically, cognitive models emphasize intolerance of uncertainty, catastrophic misinterpretation of bodily sensations, and reinforcement loops where worry temporarily reduces perceived threat but ultimately increases anxiety through avoidance and attentional bias. Worry can function as a coping strategy (mental rehearsal, problem-solving, rumination), yet it perpetuates anxiety by maintaining an availability of feared outcomes and preventing corrective learning. Behavioral avoidance, safety behaviors, and reduced engagement with rewarding activities further maintain symptoms and can lead to secondary depressive symptoms.

First-line psychotherapeutic approaches include Cognitive Behavioral Therapy (CBT) with a focus on restructuring maladaptive beliefs, reducing worry time, enhancing cognitive flexibility, and addressing avoidance. CBT often incorporates exposure to feared situations or sensations, interoceptive work, relaxation training, and skills for managing intolerance of uncertainty. Meta-analytic evidence supports the effectiveness of CBT, with durable gains for many patients.

Pharmacologically, selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used due to their favorable efficacy and long-term safety profiles. Treatment generally requires several weeks for full benefit, and partial response is common early in therapy. Clinicians consider dosing strategies, comorbidity targets (e.g., depression), side-effect tolerability (e.g., gastrointestinal effects, insomnia, sexual dysfunction), and drug interactions. Short-term benzodiazepines may be used cautiously for acute symptom relief, but they carry risks including sedation, dependence, tolerance, and cognitive impairment; therefore, they are usually not a stand-alone long-term solution.

Lifestyle and adjunctive strategies can support symptom reduction. Sleep optimization, caffeine and stimulant moderation, regular aerobic activity, mindfulness-based interventions, and stress management may improve baseline arousal and reduce symptom intensity. However, these interventions generally complement rather than replace CBT or pharmacotherapy for moderate to severe GAD.

Clinically, risk management includes evaluating suicidal ideation, functional decline, and substance misuse. Persistent anxiety can impair work performance, relationships, and physical health through chronic stress physiology.
