These agents require time to achieve full effects and may initially increase anxiety in some patients, necessitating careful titration.

Benzodiazepines can reduce acute anxiety symptoms via GABA-A receptor modulation and may be used short term in select cases; however, risks include sedation, falls, cognitive impairment, dependence, and withdrawal, so they are generally not preferred for long-term management. Buspirone, pregabalin, and other agents may be considered for specific indications depending on local guidance and patient factors. For phobic disorders, exposure-based therapies are particularly effective, gradually extinguishing fear responses through repeated, controlled contact with the feared stimulus.

Adjunctive strategies improve outcomes: sleep regularity, reduction of stimulant intake, aerobic exercise, mindfulness-based techniques, and stress management. Clinicians should also address comorbidities such as depression, substance use disorders, and chronic medical illnesses. With consistent therapy and appropriate medication when indicated, many patients experience meaningful symptom reduction, improved functioning, and enhanced quality of life.
