
Groin pain in athletes commonly reflects injury to the adductor muscle group and surrounding structures at the hip and pelvis. The term “groin issue” is often used clinically to describe adductor strain, tendinopathy, or less commonly femoroacetabular/inguinal pathology. In football and similar sports, these injuries are driven by rapid acceleration and deceleration, high-intensity cutting, kicking, and repeated hip flexion with adduction. Biomechanically, the adductors act to stabilize the pelvis during single-leg stance and to generate force during lateral movements; when loading exceeds tissue capacity, microtrauma accumulates, leading to partial tearing, tendinous degeneration, and pain.
Adductor strain typically presents as an acute onset pain in the inner thigh/groin during sprinting, twisting, or kicking. Athletes may describe a sudden “pull,” localized tenderness, weakness, and reduced range of motion. In more severe cases, bruising and palpable defects can occur. Subacute or chronic groin pain may reflect adductor-related tendinopathy, where repetitive loading causes collagen disorganization, neovascularization, and impaired tendon mechanical properties. Differential considerations include hip joint pathology, iliopsoas tendinopathy, stress injury, nerve entrapment, and inguinal hernia; accurate localization is critical because management differs.
Diagnosis begins with history and targeted physical examination. Clinicians assess pain provocation during resisted adduction, hip adduction range, palpation of the adductor origin and tendon, and functional testing such as squeeze tests and sport-specific movement screens. Imaging is used to stage injury and guide prognosis. Ultrasound can detect tendon tears and fluid around the adductor complex, while magnetic resonance imaging (MRI) provides detailed evaluation of muscle fiber disruption, tendon integrity, bone marrow edema, and differential diagnoses. MRI can classify severity by extent of muscle injury and involvement of tendon and nearby structures, which correlates with time to return.
Treatment prioritizes pain control, restoring tissue capacity, and preventing re-injury. Early management focuses on relative rest from aggravating activities rather than complete immobilization. Cryotherapy and analgesia may help symptom management, but long-term recovery depends on progressive loading. Evidence-informed rehabilitation generally includes: (1) phase-based isometrics to reduce pain and maintain strength, (2) gradual introduction of concentric and eccentric strengthening to improve tendon/muscle capacity, and (3) dynamic, sport-specific drills that recreate high-demand adduction and hip stabilization. Eccentric training is particularly important for tendon remodeling, improving collagen alignment and load tolerance. Neuromuscular control work—core stability, pelvic control, and movement retraining—addresses the common mechanistic contributors of groin recurrence.
Return to play decisions should be criterion-based. Common objective measures include pain-free resisted adduction strength, symmetrical range of motion compared with the contralateral side, and functional tests such as adduction squeeze strength, sprint readiness, and controlled cutting performance without symptom escalation. A typical phased timeline depends on grade: mild strains may recover faster, while higher-grade injuries involving significant fiber disruption or tendon involvement require longer rehabilitation. Importantly, premature return is associated with higher recurrence risk because tissues may still be in a vulnerable remodeling phase.
Prognosis is influenced by injury grade, previous groin injuries, strength deficits, hip mobility limitations, and lumbopelvic control. Risk mitigation strategies include preseason screening for adductor strength imbalances, progressive preseason conditioning emphasizing hip adduction capacity and eccentric control, and workload management to avoid sudden spikes in training volume or sprint frequency. Technique and tactical demands should be considered, since frequent cutting and kicking can perpetuate overload.
Although the athlete’s context may be described as “back soon,” clinicians translate this into a measurable plan. Clear communication between medical staff, physical therapists, coaches, and the athlete supports safe progression. If pain persists beyond expected recovery windows or if symptoms worsen despite rehabilitation, reassessment is warranted for overlooked diagnoses such as stress injury, labral pathology, or hernia.
