If perforation is suspected, treatment escalates to broad-spectrum antibiotics, nil per os (nothing by mouth), close monitoring, and surgical or interventional management depending on extent.
Prevention centers on reducing foreign-body risk. Thorough chewing remains the primary protective factor. For fish, practices such as careful boning, using fillets, and visually inspecting for bones can markedly lower incidence. Eating slowly, taking smaller bites, and avoiding eating when distracted or impaired by alcohol or sedation reduces aspiration and bolus mismanagement. Individuals with dysphagia, neurological impairment, prior head and neck radiation, or known esophageal narrowing should follow tailored dietary plans and may benefit from swallow therapy.
There is also an adjacent nutrition consideration: eating “unpeeled oranges” implies retained fibrous material, which is usually safe but can be harder to chew for some people.
