Cognitively, many anxiety disorders are maintained by biased threat appraisal and intolerance of uncertainty. For example, in GAD, chronic worry functions as a cognitive strategy to attempt to prevent negative outcomes; however, it paradoxically sustains anxiety by preventing corrective learning. In panic disorder, catastrophic misinterpretation of interoceptive sensations (e.g., dizziness, palpitations) leads to fear of fear, creating a feedback loop that can culminate in panic attacks. In social anxiety disorder, negative beliefs about performance and social evaluation drive hypervigilance to perceived scrutiny, along with safety behaviors (e.g., avoiding eye contact, rehearsing excessively) that prevent disconfirming evidence.
Typical clinical features vary by disorder but commonly include excessive worry, restlessness, irritability, muscle tension, sleep disturbance, and impaired concentration in GAD; recurrent unexpected panic attacks and behavioral changes such as avoidance in panic disorder; and intense fear of embarrassment or humiliation in social anxiety disorder. Specific phobias center on circumscribed triggers, while agoraphobia involves fear of situations where escape might be difficult or help unavailable, leading to avoidance and functional impairment.
Diagnosis requires careful differential assessment to distinguish anxiety disorders from medical conditions and substance-induced states. Hyperthyroidism, cardiac arrhythmias, pulmonary disease, medication effects (such as stimulants), and withdrawal syndromes can mimic anxiety. Substance use (including excessive caffeine) can also exacerbate symptoms. Clinicians also evaluate comorbidities, which are common: major depressive disorder, obsessive-compulsive disorder, posttraumatic stress disorder, and substance use disorders frequently co-occur. Standard diagnostic criteria rely on symptom duration, severity, and the presence of impairment or clinically significant distress.
Evidence-based treatment is multimodal and often includes psychotherapy, pharmacotherapy, or both. Cognitive behavioral therapy (CBT) is first-line for many anxiety disorders and targets maladaptive thought patterns and avoidance behaviors. CBT techniques include cognitive restructuring, interoceptive exposure for panic, graded exposure for phobias, and prevention of safety behaviors to facilitate extinction learning. For GAD, CBT frequently incorporates metacognitive strategies, problem-solving skills, and worry-time scheduling. Mindfulness-based approaches and acceptance-oriented interventions can complement CBT by reducing experiential avoidance and altering the relationship to intrusive thoughts.
Pharmacotherapy may be indicated for moderate to severe symptoms or when psychotherapy is insufficient. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used and have evidence for multiple anxiety disorders. Benzodiazepines can reduce acute anxiety but are generally reserved for short-term or bridging treatment due to risks of sedation, dependence, and cognitive impairment. Buspirone may be used for GAD in selected patients. Choice of medication should consider comorbid depression, age, pregnancy status, drug interactions, and patient preference.
Treatment response is monitored with symptom scales, functional assessment, and evaluation of relapse risk. Relapse prevention emphasizes sustained exposure practices, ongoing cognitive restructuring, and management of stressors that trigger relapse. Lifestyle factors—sleep regulation, limiting alcohol, reducing caffeine, and engaging in regular physical activity—support recovery but are not replacements for targeted therapy.
In summary, anxiety disorders reflect dysregulated threat processing, altered inhibitory control, stress system variability, and maladaptive cognitive-affective loops.
