Physical examination and targeted labs are selected based on history and red flags. This step is essential to ensure that treatable physiologic drivers are not mistaken for primary anxiety pathology.

Evidence-based treatment is typically stepped, multimodal, and individualized. Psychotherapy is a first-line option. Cognitive behavioral therapy (CBT) targets maladaptive beliefs and avoidance, using cognitive restructuring, behavioral experiments, and graded exposure. For GAD, CBT frequently integrates worry management, cognitive techniques, and interoceptive awareness strategies. Exposure-based interventions are particularly effective for phobias and social anxiety, leveraging inhibitory learning—relearning that feared cues are not followed by catastrophic outcomes. Relaxation training and mindfulness-based strategies can reduce arousal and improve emotion regulation, though they are most effective when integrated into a broader CBT or exposure framework.

Pharmacotherapy may be indicated for moderate to severe symptoms or when rapid symptom reduction is needed. SSRIs and SNRIs are commonly used due to favorable efficacy and tolerability profiles across multiple anxiety disorders. Dosing often requires several weeks for full effect, and patients are monitored for activation, sleep changes, or gastrointestinal effects. In selected cases, short-term use of benzodiazepines may be considered for acute distress; however, risks include sedation, cognitive impairment, falls, and dependence, so long-term use is generally avoided. For specific patients with treatment-resistant anxiety, augmentation strategies may include buspirone, pregabalin (where appropriate), or other evidence-informed options, guided by specialist assessment.

Overall prognosis is variable but improved by early diagnosis, sustained engagement in therapy, and management of comorbid conditions.
