The fatal accident involved a red 1994 Honda Gold Wind SE driven by Cox, who was slowing down to make a left turn onto TR 454A when he was rear-ended by a silver 2002 Pontiac Grand AM operated by Raven Kolton-Lee Reynolds, a 21-year-old from West Lafayette. The impact of the collision caused the Honda to veer off the road, striking a tree before coming to a stop down an embankment.
