The Tragic Death of a Defence Force Service Person
In a recent ruling, a coroner has determined that the death of a Defence Force service person at a New Zealand military base was self-inflicted. The individual, whose name and identifying details have been suppressed, had been engaging with mental health services both before and during their enlistment with the NZDF. At the time of their tragic death in 2013, they were also the subject of an internal investigation.

Coroner’s Recommendations for Change
Following a hearing in 2023, Coroner Marcus Elliot has released a series of recommendations to address the circumstances surrounding the service person’s death. These recommendations have been categorized into investigations, support, and information sharing.
According to the coroner, when conducting internal investigations that could potentially jeopardize the status of personnel, the NZDF should ensure that the individual under investigation is adequately supported. Elliot also emphasized the importance of addressing mental health issues during investigations, recommending that medical officers overseeing individuals with mental health concerns identify potential stressors that may increase the risk of suicide and develop plans to address them. Support should be offered during stressful times, such as interviews, and mental health risks should be considered in operational decisions.
Changes and Progress in Mental Health Support
While acknowledging that the NZDF has implemented various changes since 2013, Elliot highlighted the need for ongoing improvements in mental health support within the organization. The NZDF Mental Health Strategy and implementation plan, developed with input from medical and mental health professionals, have been periodically refreshed to enhance support for service members.
