The death of a nurse who developed cancer and died three years after having a cyst removed because she was mistakenly given the all-clear was described as “avoidable” during an inquest. Catherine Jones, a nurse at Wrexham Maelor Hospital, underwent surgery to remove an ovarian cyst in July 2013. However, it was revealed during a previous hearing that a biopsy sample from the cyst should have been identified as borderline cancerous. This meant that Mrs. Jones would have been offered surgery to remove the cancerous ovary, increasing her chances of survival. Instead, she was given the all-clear.
Unfortunately, the cancer returned in the summer of 2016, and Mrs. Jones was rushed back to the hospital. Tests revealed a cancerous tumor, and she underwent a hysterectomy. Despite being initially told she was all-clear, it was later discovered that the cancer had spread, and she died a few months later.

The ongoing inquest, set to last four days, is examining issues related to Mrs. Jones’ consultation and treatment. In a statement read during the inquest, Catherine’s mother expressed her daughter’s concerns about her care and a “catalogue” of other issues. Retired consultant Philip Toon confirmed that an ultrasound scan had identified the cyst in 2012, and Catherine had expressed a desire to have it removed. The cyst was removed in 2013, and a biopsy indicated no evidence of malignancy. However, it was later acknowledged that there had been a fundamental misunderstanding, and the recommended surgery would have prevented Mrs. Jones’ death.
The impact of this mistake on the case has been described as “catastrophic” by Louis Browne QC, who is representing Mrs. Jones’ family. The inquest is currently ongoing at Ruthin Coroner’s Office.
